Strategic Engagement with Council/Clinical Commissioners and Local Care Organisation.

Local Care Organisation purpose

Local Care Organisation Bury is the partnership between Clinical Commissioners and Bury Council, it overseas the commissioning of services dedicated to health and well-being.

Integrated Neighborhood Teams

The integrated neighborhood teams are the placed based locality working teams that will deliver the Health and Social care transformation in Bury. There will be 5 teams within Bury and they will establish links with voluntary and community organisations to deliver client based care to residents.

Multi-Disciplinary Teams Locality based

The MD Teams are GP led clinical teams within Adult health and Social care leading the transformation of health and well-being for the top 10% of patients accessing Acute service. Each teams will support patients to access community based services to develop resilience and well-being in a more holistic and person centered manner. 

The Role of the MDT will use a risk stratification tool to identify those in need and support  their transition to health.
